Physiotherapy - self referral

https://www.smartsurvey.co.uk/s/Physiotherapy-SelfReferral/There is a new physiotherapy self-referral service at the Princess Royal Hospital.  This will mean that you do not need to see your doctor before contacting the Physiotherapy Department.  You must be aged 16 years to be seen by the SMSKP Physiotherapy Service.  If you are under 16, please contact your GP for advice. 

The benefits of self-referral include:

- saving time for both GPs and patients

- reducing waiting times

- improving attendance levels at appointments

- empowering patients to manage their condition

The referral will be treated in the same way as if your GP had referred you. 

You may be telephoned to discuss your referral and they may be able to provide advice and exercises over the telephone.

Please either complete the self-referral form, giving as much information as possible, filling it out honestly to give a clear understanding of your condition.
