Children

Children up to the age of 12 can have Patient Access Proxy consent, to allow a parent/guardian to have the ability to use this online service.   At the age of 12, the Proxy Access link will be severed and the patient will need to apply for Patient Access in their own right (ie give consent and sign the form).  Each Proxy Consent application will have to be approved by a Doctor.
