How to sign-up
For more information, please contact reception or next time you visit the surgery, bring with you a piece of photo ID (passport, driving licence) and something with your address on like a bank statement but not a gas or electric bill. If you do not have these, please talk to us and we will help you sign-up
Children up to the age of 12 can have Patient Access Proxy consent, to allow a parent/guardian to have the ability to use this online service. At the age of 12, the Proxy Access link will be severed and the patient will need to apply for Patient Access in their own right (i.e. give consent and sign the form). Each Proxy Consent application will have to be approved by a Doctor.
